Ripple Mattress vs Mattress Topper vs Memory Foam

Published 1 June 2026Last updated 19 August 2026

A ripple mattress actively alternates air pressure with a pump. A mattress topper and memory foam only spread weight passively and never remove pressure from any one point. For a patient who can still shift position, a topper may be enough. For a patient who cannot reposition themselves, passive surfaces do not prevent pressure ulcers because the load never leaves the skin. Water mattresses redistribute weight but are heavy, hard to move and give no cyclic relief. Only alternating pressure removes load on a cycle.

Short answer

A ripple mattress actively cycles air through alternating cells using a pump, so pressure on the skin changes every few minutes. A memory foam or gel topper is a passive surface that spreads weight over a wider area but never removes load from any point. For a patient who cannot shift their own weight, or who already has a sore, the ripple mattress is the stronger choice because only active alternation fully unloads skin. A foam topper suits patients at lower risk who still move themselves, settings without reliable power, or someone who mainly wants comfort rather than clinical pressure relief.

Why memory foam is the wrong tool for a fully immobile patient

Memory foam has a real advantage: it conforms to the body and spreads weight over a larger surface, which lowers peak pressure at any single point. But lowering pressure is not the same as removing it. If a patient cannot shift their own weight, the same skin stays loaded for hours, and tissue damage depends on how long pressure lasts as much as how hard it is. Foam also traps heat, and a warmer, sweatier skin surface breaks down faster, which matters in Kenyan conditions. For a patient who is unconscious, paralysed or fully immobile, foam alone is not adequate.

When a topper is the right answer

Choose a foam or gel topper if the patient still turns themselves in bed, is on short-term bed rest with healthy skin, is at low risk on a clinical assessment, or lives somewhere with unreliable power where a pump would sit idle much of the time. A topper is also a reasonable complement rather than a substitute: some families use a ripple mattress for the patient and a foam topper on the caregiver's own bed, which is a fair split of the budget. If the goal is genuinely comfort rather than pressure care, the topper is the better purchase and the ripple mattress will disappoint.

Can you use both together?

Not stacked. Putting a memory foam topper on top of a ripple mattress cancels the ripple mattress entirely, because the foam layer absorbs the alternation before it reaches the skin. This is a common and expensive mistake. The ripple mattress goes on top of the base mattress, and nothing but one thin cotton sheet goes on top of it. If you already own a foam topper, put it under the ripple mattress or on another bed.

What about hospital beds and standard beds?

All three options lay on top of an existing mattress and work on both standard domestic beds and adjustable hospital beds. The ripple mattress needs enough tube slack to move with the frame when the head is raised, so check that when you set it up. Foam and gel overlays have no such constraint but can slide on a tilting frame, so secure them with the corner straps if fitted.

Common questions

Is a memory foam topper enough to prevent bedsores?

For a patient who still repositions themselves and has healthy skin, it can be. For a fully immobile patient or anyone with an existing sore, it is not, because it lowers pressure without ever removing it.

Can I put a foam topper on top of a ripple mattress?

No. The foam absorbs the alternating pressure before it reaches the skin and cancels the benefit. Use only one thin cotton sheet over a ripple mattress.

Which is better for a patient who sweats a lot?

The ripple mattress. Air moving under the body reduces heat and moisture, whereas memory foam traps both.

Which is cheaper overall?

Foam toppers start lower, but a treated pressure ulcer costs far more in dressings, nursing and hospital time than the difference between the two.

Does a ripple mattress work without power?

It holds air and works as a static surface during an outage, but the alternating benefit stops. In areas with long frequent outages, plan for a small inverter or UPS.
